Notes About Your First Visit
Please bring the following with you to your appointment:

Medical Insurance Card(s)

Current list of medications, including dosages.

Medical power of attorney paperwork (if applicable).

If you have a co-payment, insurance deductible balance, or elect to have surgery, you may be asked to make a payment during your appointment. Please have a form of payment with you for this purpose. We accept cash, check, Visa, MasterCard and CareCredit. We also gladly accept Medicare assignment.
Appointment times vary depending upon the type of evaluation. Your appointment may last up to two hours.
If considering cataract surgery, please discontinue any contact lens wear a minimum of two weeks prior to your appointment.
Not that ALL new patients will be dilated, thus we strongly recommend you bring a driver with you to your appointment
If your medical decisions are made by your Medical Power of Attorney, that person must be present at your visit.
